"Health Index. Ukraine". Presentation of KIIS research data

Kyiv, December 21, 2018 - The Ukrainian Health Crisis Media Center hosted a presentation of the results of this year's “Health Index. Ukraine”, initiated by the International Renaissance Foundation, the Kyiv International Institute of Sociology and the School of Public Health of the National University of Kyiv-Mohyla Academy. The purpose of the study is to determine the actual level of satisfaction of Ukrainian citizens with medical care, as well as their experience and behavior in health-related issues. The research is conducted for the third year in a row and allows us to trace changes in the experience, perceptions and attitudes of Ukrainians to individual components of medical reform

The study data was presented by the deputy director of KIIS Victoria Zahozha.


In 2018, the national value of the Health Index was 62.2 points (in 2017 - 62). In addition to satisfaction with medical assistance, the treatment of a doctor and the prevention of diseases, the cost of drugs and the refusal to consult a doctor due to lack of funds, also include: vaccination of children, knowledge of stroke symptoms, self-assessment of health state, overweight or obesity .

Ambulatory unit

According to the study, in 2018, the high cost of treatment was a financial barrier for applying for outpatient care for 17% of those polled (2017 - 22.9%, 2016 - 24.6%). In general, in Ukraine, 76% of citizens are satisfied with district medical practitioner / family physician (2017 - 73%). The least satisfied are in the Sumy (37%) and Kirovograd (54%) regions, the largest - in Volyn (96%) and Kharkiv (91%) regions; a similar distribution across regions was observed last year.

"Satisfaction with medical care is a subjective assessment. However, it should be borne in mind that "money is for the patient" is the basis of health care financing reform. Therefore, it is precisely such a subjective assessment that will determine to which medical institution eventually comes the patient - obviously, to the one that he is most satisfied with, "- comments Tetyana Steporko, research coordinator and assistant professor of the School of Health.

In 2017 Ukraine introduced the "Available drugs" program, which, among other things, was aimed at providing patients with hypertension with drugs with proven effectiveness. Survey data of 2018 show that about 40% of patients with hypertension have benefited from the "Available drugs" program (in 2017, those were less than 21%).

The average cost of drugs for the last 30 days - 572 UAH (in 2017 - 570 UAH), with the most money spent by residents of Ternopil region (1103 UAH) and Poltava region (1029 UAH), the least - Kherson region (356 UAH) and Dnipropetrovsk region (364 UAH .) It is worth noting the sharp drop in this indicator in Dnipropetrovsk region compared with 2017 (then it was 883 UAH).

To the question of those respondents who were hospitalized, "How much was it  difficult for you and your family to find the means to cover all the costs (formal and informal) associated with this inpatient treatment: it is impossible, difficult or easy?" 48% They said that it was difficult or impossible for them to find the money. For comparison, in 2017 and 2016 the insolvency was somewhat higher - 60% and 58% respectively. That is, in general, in the year 2018, the financial availability of inpatient care has improved somewhat. Although some segments of the population have costs exceeding their monthly income: the total cost of hospitalization in the last 30 days was a significant financial burden for patients, as they accounted for 110% of their household income per month (53% in 2017).

To the question "During the last 12 months, how many times have you been ill and needed inpatient care but have not been hospitalized at all due to lack of funds?" In 2018, 9% of respondents said that they refused hospitalization, whereas in 2017 and 2016, - 12% and 32% respectively. Thus, the financial burden of inpatient care has become smaller over the past three years.The most problematic aspect of inpatient treatment is "drug availability": in 2018 66% rate it as "bad". In general, in Ukraine, 56% of the polled respondents are satisfied with medical assistance in the hospital. The least satisfied are in the Sumy (28%) and Khmelnytsky (29%) regions, the most in Kherson (86%) and Kharkiv (78%) regions.

Ukraine"is a unique source of data, since for three consecutive years, it systematically monitors the attitude, experience and behavior of health care users and those who do not use it. These data not only illustrate how the implementation of medical reform in different regions affects health care users, but also helps to make informed decisions for all those involved in the development of health policies at the national and local levels, " said Victoria Timoshevskaya, director of the Public Health Program Initiative of the International Renaissance Foundation.

Ukraine" has several significant benefits: a large sample size (this allows you to reproduce a representative picture for both adult health care users of different levels and for those who for some reason do not turn to medical professionals); the representativeness of the survey results for each individual region of Ukraine (which can stimulate interregional exchange of positive practices); the regularity that makes it possible to see a change in the situation over time, and whether these changes are in line with expectations. All this distinguishes research among others covering the issues of the use of medical services in Ukraine ", - said Victoria Zakhozha, deputy director of the Kiev International Institute of Sociology.

 

Research methodology

The field study was conducted by the Kyiv International Institute of Sociology in cooperation with the Social Indicators Foundation, thanks to the support of the International Renaissance Foundation during June-July 2018. The sample designed for the study is random and representative for an adult (aged 18 years and over) of Ukraine's population in general and each of the 24 oblasts of Ukraine and Kyiv. In the Donetsk and Luhansk regions, the study covers only territories under the control of the Government of Ukraine. As a result of the field stage, 10,194 questionnaires were collected and at least 400 respondents were interviewed in each oblast. The questionnaire of the study was co-ordinated by the international Scientific Council of the project. In 2018, the study was conducted with the support of the World Bank Joint Project of the Ministry of Health of Ukraine "Improving Health at the Service for People".
